I hereby authorize the staff at Rockdale Studios LLC to act for me according to their best judgment in any emergency requiring medical attention, and I hereby waive and release Rockdale Studios LLC from any and all liability for any injuries or illnesses incurred while attending. I understand that participation in Rockdale’s programs will involve the use of electrical equipment which may carry with them the risk of injury. Rockdale Studios LLC is not responsible for personal items that are lost, stolen or damaged. All medical expenses incurred will be the responsibility of the student or student’s family.

I also understand that Rockdale Studios LLC retains the rights to use any photographs, videos, songs, recordings, or any other record of the event for publicity, advertising, or any legitimate purpose. I further agree to indemnify and hold harmless Rockdale Studios LLC, their teachers and agents from and against all liability, save and except for the sole negligence of Rockdale Studios LLC, resulting in injury associated with this individual’s participation in the program and understand that all terms are subject to change.
